New 'Deadpool 2' details released following on-set death

Five days after stunt woman SJ Harris was killed on set while shooting a scene for Deadpool 2 in Vancouver, British Columbia's workplace safety agency released the first official account of what happened.

Harris, 40, had been rehearsing a stunt scene "that involved driving a motorcycle out of the open doors of a building, across a concrete pad and down a ramp that had been built over three stairs and coming to a stop on the stair landing," read the official WorkSafeBC report obtained by USA TODAY.

The report says during the first shooting of the scene, Harris, a stunt double for actress Zazie Beetz, continued driving beyond the planned stopping spot on the stairway landing, continued down a second ramp and across the road.

"The motorcycle struck the concrete sidewalk curb, (and Harris) was thrown off the motorcycle and propelled through a plate glass window of a building," read the report. She died on the scene.

Harris was riding a Ducati Hyperstrada 939.

Production of Deadpool 2 went dark for two days following the stunt woman's death. Star Ryan Reynolds held a moment of silence with the crew before cameras rolled Wednesday.

The actor released a statement Monday saying the entire cast and crew were "heartbroken, shocked and devastated" over the loss.

Last month a stuntman for The Walking Dead died after suffering serious injuries on the Georgia set. John Bernecker, 33, fell more than 20 feet onto concrete from a balcony while shooting the AMC show. An investigation by the U.S. Department of Labor's Occupational Safety and Health Administration remains open.

On Wednesday it was confirmed Tom Cruise broke his ankle during a roof-jumping stunt on Mission: Impossible 6, an injury which has forced a temporary shutdown of the major film's production.
